Individuals diagnosed with HD or JHD

Immediate family caregivers of someone diagnosed

Advocates applying on behalf of a warrior (with consent)
All applicants must reside in the U.S. and provide documentation of diagnosis.
What to Expect
Our application forms will walk you through a few easy steps:

Tell us who you are and what you need

Submit proof of diagnosis (or doctor’s note)

Include photos or additional documents if needed
We review every request with care. Decisions are typically made within 2–3 weeks, depending on the volume of requests.
